False Report to the Health Fund

I went on a women's vacation with the understanding that I would receive reimbursement from the health fund for emotional treatments for my child. They gave me a "receipt" stating we were in treatments, and later I realized that the institute that provided the receipt requires a letter stating that I am very satisfied with the therapist and therefore went specifically to her - I have never seen her and have no idea who she really is. If I don't submit, I will lose a 1000 shekel reimbursement. Is it permissible according to Halacha to submit such a letter?
