The text is as the Mishnah Berurah writes (siman 110, se’if katan 35), and this is his wording:
It is good to say one short text that includes much, and this is it:
May it be Your will, Hashem, our God and the God of our fathers, that You illuminate my eyes with the light of Your Torah, and save us from every stumbling block and error, whether in the laws of what is forbidden and permitted, whether in monetary laws, whether in rendering halachic rulings, whether in learning. “גַּל עֵינַי וְאַבִּיטָה נִפְלָאוֹת מִתּוֹרָתֶךָ” — “Uncover my eyes, that I may behold wonders from Your Torah.” And regarding that in which I have already erred, set me upon the truth; and do not remove from my mouth a word of truth, exceedingly, for “כִּי ה' יִתֵּן חָכְמָה מִפִּיו דַּעַת וּתְבוּנָה” — “Hashem gives wisdom; from His mouth come knowledge and understanding.”
And when one learns in a group, he must also request that he not rejoice in their stumbling, and that they not rejoice in his stumbling, as is stated in the Gemara.
