How many misdeeds (halachic) would warrant a need for an intervention in an adolescent whose parents wish for them to be observant

I am not sure if I understand your question.

Every situation is different and much will depend on the child and the child’s relationship with his parents. No two children are the same.

Parents are to always encourage their children to follow the mitzvot of the Torah. When a parent sees a child violating the Torah the parent should gently speak to about it to the child in a loving manner. However extra sensitivity needs to be taken when the child is doing it as a form of rebellion, and one should seek advice from professional educators on how to deal with it.
