Yom Kippur Fast - Diabetes, Heart Attacks, and a Stroke

Question

Sorry, I am asking for the second time because my previous email got stuck and I did not receive the answer. I am 53 years old, have been suffering from diabetes for about 12 years and inject insulin, I have had 5 heart attacks, 5 stents, and one bypass surgery which I underwent about 3 and a half years ago, the last attack was on the last fast of Esther (about half a year ago). About 3 years ago I had a mild stroke and I also had 4 kidney stone crushing procedures, in the last two years there are no stones, thank G-d. I take medications for the heart, blood pressure, kidney stones, etc. The list of medications: Cardilock, Umperdex, Cartia, Aluril, Urocit-K, Azkor, Tritace, Litorva 80, Hapient, Magnog, and of course I inject Humalog insulin between 20 to 30 units as needed and sometimes several times a day. My question is how to behave on Yom Kippur regarding food / drink etc.
