Yom Kippur Fast - Heart Attacks and a stroke

Question

I have been a diabetic for about 10 years and inject insulin. I’m after 5 heart attacks, 5 stents, and one bypass surgery (the surgery was about 3 years ago). The last heart attack was on the Fast of Esther last year. I had a mild stroke about 3 years ago, a few months after the surgery, and I have undergone 4 kidney stone removals. I am being treated with medications for stones, heart, blood pressure, etc. In addition, I inject insulin for diabetes. My question is, how should I behave on Yom Kippur regarding eating/drinking?
